Incidence and Predictors of Vascular Complications after Invasive Coronary Procedures: A Prospective Analysis |
| Neelima Penugonda, MD, Jennifer Jones, MD,
J. Richard Spears, MD, Theodore Schreiber, MD
Wayne State University, Detroit, Michigan |
Background. Although clinical trial data suggest that the incidence of hemorrhagic vascular complications associated with invasive coronary procedures is relatively low, contemporary data from tertiary care centers are lacking.
Objective. The purpose of the study was to determine the incidence and potential predictors of hemorrhagic vascular complications after invasive coronary procedures, either diagnostic alone (contrast angiography [CA]) or combined with an interventional procedure (percutaneous coronary intervention [PCI]).
Methods. Clinical characteristics and procedural data in 200 consecutive patients undergoing an invasive coronary procedure (CA = 124; PCI = 76) at a single large urban tertiary care center were collected prospectively during their hospital stay. Multivariate logistic regression methods (SAS v9.1) were used to identify independent predictors of hemorrhagic vascular complications.
Results. The overall incidence of hemorrhagic vascular complications was 10%

Excerpt from: Chapter 19. STEMI Interventions – Future Perspectives |
| Sameer Mehta, MD, FACC, MBA |
Dr. Mehta will be commenting on the important work going on around the world as societies and their hospitals struggle to educate patients about the importance of timely intervention in ST-elevation myocardial infarction, and work collaboratively to decrease the time from patient arrival to intervention.
This month, we share an excerpt from Dr. Mehta’s upcoming Textbook of STEMI Interventions.

A 3D CT Vessel “Roadmap” Over Live Fluoroscopy for Chronic Total Occlusion |
| Cath Lab Digest talks with Jeffrey W. Moses, MD |
During a live case at TCT 2007, Dr. Jeffrey Moses demonstrated a unique new technology currently under development by GE Healthcare, (Chalfont St Giles, United Kingdom). Using a next-generation GE Advantage Windows workstation, a patient’s prior cardiac computed tomography angiography (CTA) images can be superimposed on live fluoroscopy as a “roadmap” for guidewire advancement, chronic total occlusion (CTO) crossing and therapy delivery. Dr. Moses discusses the increasing use of CTA, the treatment of CTOs and his thoughts on where this technology is headed.
