NIRS can help determine the distribution and chemical composition of underlying plaque, and allows for the identification of vulnerable plaques.
A paravalvular leak (PVL) closure is often extremely challenging due to the leak’s small size and the fact that its location is not visible on X-ray during the attempted intervention. 
I have always used a shorter infusion strategy and more recently, have moved to a bolus-only approach for most patients. Reducing the duration of the infusion still preserves the benefit but decreases the bleeding risk.
